Entrevistamos a María Luz Couce Pico

Hoy tenemos con nosotros a Mª Luz Couce Pico, doctora en Medicina y Cirugía. Mª Luz es pediatra y jefa de servicio de Neonatología y directora de la Unidad de Diagnóstico y Tratamiento de Enfermedades Metabólicas Congénitas del Hospital Clínico Universitario de Santiago de Compostela.

Queremos que conozcáis cómo es ser médico además de liderar un grupo de investigación que estudia varias patologías en las que está incluido el síndrome de Dravet. Mª Luz es una mujer que lucha por mejorar la vida de sus pacientes, y leyendo su entrevista podrás empatizar con ella en cómo es su día a día. ¡Te invitamos a conocerla!

INTRODUCCIÓN

¿Por qué quisiste ser médico?
Lo decidí casi en el último momento, tenía dos carreras diferentes que me gustaban, la de Medicina y la de Ciencias Exactas, y al final la faceta humanitaria de Medicina hizo que me decantase por ella.

¿Cuándo descubriste tu vocación?
Mi vocación de verdad empezó durante la carrera, y se incrementó más con el desarrollo de mi actividad profesional. Conforme avanzaron los años de mi actividad profesional, me sentí cada vez más inmersa en ella y no dudé en ningún momento de que fue la mejor opción que pude elegir.

¿Te apoyaron tus padres en tu decisión?
Mis padres no proceden del mundo sanitario, aunque tengo otros familiares que también lo están, y siempre me han respetado mis planteamientos. Fui una muy buena estudiante y quizás eso haya contribuido a dejarme carta blanca en mis decisiones.

CAMINO ACADÉMICO Y PROFESIONAL

¿Qué has estudiado?
Soy Licenciada en Medicina y Cirugía. Mi especialidad es Pediatría y, dentro de ella, me fui especializando en Neonatología y en las Enfermedades Metabólicas Hereditarias.

¿En qué consistió tu tesis doctoral?
Mi tesis doctoral, que inicié durante la Residencia de la especialidad MIR de Pediatría, trató sobre las secuelas endocrinológicas de la radioterapia craneal en niños con cáncer, la afectación sobre todo a nivel del eje hipotálamo-hipofisario.

¿Te costó llegar a ser médico?
En nuestra profesión es indispensable un esfuerzo continuo de formación y actualización. Todos mis logros profesionales los he conseguido con una combinación de trabajo y perseverancia.

¿Qué valores crees que se necesitan para ser médico?
Esfuerzo, dedicación, entusiasmo, interés, curiosidad, empatía,…

¿Qué es lo que más te gusta de ser médico?
El sentirte útil y el trato directo con el paciente. A mayores ahora que ocupo puestos de gestión, el poder definir, aunque parcialmente, una meta que intente llevar a la excelencia en la calidad asistencial.

¿Y lo que menos?
Lo difícil que es mover las ideas prefijadas de la administración, la ciencia va más deprisa.

¿Cuál es tu objetivo profesional?
La mejora continua de la actividad asistencial, y ello implica medidas de calidad con equipo multidisciplinar, investigación a alto nivel y formación óptima.

EL DÍA A DÍA EN INVESTIGACIÓN BÁSICA

¿Cuáles son las técnicas más usadas tu laboratorio?
La espectrometría de masas en tándem, métodos colorimétricos, estudio de actividades enzimáticas, estudios genéticos que incluyen exoma completo, exoma dirigido y paneles genéticos.

En tu opinión, ¿cuál es la más complicada?
Probablemente el exoma, por la dificultad de interpretación de las variantes de significado incierto.

¿Cuál es la técnica más novedosa?
No sé si más novedosa, pero quizás el transcriptoma permita ahora identificar variantes genéticas que se nos escapaban y con ello ampliar el conocimiento de esta patología.

¿Y la más «antigua»?
La secuenciación por Sanger.

¿Cuánto tardas en procesar una muestra?
En general, inferior a un mes, aunque no hay tiempo exacto.

¿De dónde consigues las muestras?
De profesionales que nos las envían.

¿Qué proyectos tienes para este año?
– Un estudio genético rápido en trío de exoma y transcriptoma en enfermedades neonatales de base genética.
– Estudio de marcadores de detección de la enfermedad de Fabry.
– Un estudio sobre una enzima encapsulada en nanopartículas que atravesará la barrera hematoencefálica.
– Varios ensayos clínicos de terapia génica en fase I/II.

¿Has tenido que interrumpir algún proyecto porque los resultados no eran los esperados?
Si, uno en 2010 con la triheptanoína en trastornos de la β-oxidación de ácidos grasos de cadena larga por los problemas surgidos con la formulación.

EL DÍA A DÍA EN INVESTIGACIÓN CLÍNICA

¿Cuántos pacientes puedes llegar a ver en un turno?
Habitualmente vemos una media de 15- 20 pacientes con enfermedades metabólicas congénitas en cada mañana de consulta.

¿Se te hace difícil explicarle al paciente o a su familia la enfermedad que padece?
Sí, siempre es enormemente difícil comunicar a un paciente o a sus familiares que se padece una enfermedad grave, de carácter regresivo, fatal o que comporta el riesgo de descompensaciones potencialmente graves con posibilidad de secuelas irreversibles, especialmente si no se dispone de ninguna opción de tratamiento curativo.

¿Ha cambiado mucho el diagnóstico para el síndrome de Dravet desde que empezaste?
No es de las enfermedades en que más se ha avanzado, y queda aún mucho camino por recorrer.

¿Cuál ha sido la experiencia médica más compleja que has vivido?
Siempre lo peor es cuando fallece un paciente y no se ha conseguido lo que uno esperaba, son momentos muy duros a los que uno lógicamente no se habitúa.

¿A veces te sientes afectada por los casos de tus pacientes?
Indudablemente es imposible no interiorizar en algunas circunstancias el sufrimiento o la realidad de algunos pacientes y/o sus familias.

¿Cuáles son los mejores hábitos personales cuando se cuida de un paciente con síndrome de Dravet?
Aunque sea muy difícil de llevar a la práctica, es importante que los cuidadores puedan disponer de un período de tiempo para descansar, realizar otras actividades o tengan ocio. Este cuidado de los cuidadores es esencial para garantizar una atención de calidad de forma continuada a largo plazo a estos pacientes.

SOCIEDAD E INVESTIGACIÓN

¿Cuál es la importancia de la investigación en la sociedad?
La investigación es uno de los motores que hace avanzar a la sociedad, que nos obliga a plantearnos nuevas preguntas y buscar sus respuestas, y que puede ayudar a mejorar la vida y la salud de las personas desde múltiples ámbitos, como ha puesto de manifiesto la pandemia por COVID.

¿Crees que es importante la investigación de enfermedades raras?
La investigación en enfermedades raras es esencial porque el escaso número de pacientes afectados no las hace un objetivo prioritario de los laboratorios farmacéuticos. Los incentivos económicos instaurados por la Comunidad Económica Europea para favorecer la investigación en enfermedades minoritarias buscan paliarlo y, aunque todavía se necesita un impulso mayor a la investigación en este campo, ya se aprecian los resultados de estas medidas con un incremento destacado en el desarrollo de medicamentos huérfanos en los últimos años.

¿Qué te llevó a convertirte en miembro del Comité Científico de la Fundación Síndrome de Dravet?
Tuvo la asociación de pacientes la gentileza de ofrecérmelo y fue para mí un orgullo el aceptarlo.

¿Qué beneficios personales obtienes de esta membresía?
Poder ayudar a los pacientes, que es una satisfacción grande cuando se consigue.

PARA TERMINAR

¿Eres optimista en la lucha contra el síndrome de Dravet? ¿Por qué?
Sí, porque en los últimos años se han producido avances en el tratamiento del síndrome de Dravet como la opción terapéutica del cannabidiol que, por su acción moduladora de varios receptores cerebrales, ha demostrado disminuir la frecuencia de las crisis. A corto plazo las expectativas se centran en el STK-001, un oligonucléotico sin sentido, diseñado para regular al alza la proteína Nav1.1, reestableciendo niveles fisiológicos de la misma y potencialmente disminuyendo el número de crisis y las comorbilidades del síndrome.

TERMS & CONDITIONS

The Dravet Syndrome Conference 2023 is organised by Dravet Syndrome Foundation Spain, whose registered office is located at Calle Doctor Fleming, 30, 28036 Madrid, Spain.

The conference shall take place at the Official College of Nursing of Madrid, at Avenida de Menéndez Pelayo, 93, 28007 Madrid, Spain, on Thursday 23rd of March 2023. Attendance to this conference is free of charge, but mandatory for organisational reasons.

Participants must register online at www.dravetconference.com. Upon registration, each participant will receive an electronic confirmation by email. On-site registration will not be be possible. If a participant has not received the official email registration confirmation within 3 days, they must contact the conference secretariat at [email protected].

The conference registration includes: Attendance to all sessions, oral and visual presentations, all coffee and lunch breaks, and conference documentation with all information about the event. The registration does not include travel and accommodation costs or any other personal expenses.

Admittance to the conference without registration cannot be granted.

Conference Attendance
Upon arrival at the Official College of Nursing of Madrid the day of the event, all participants have to check-in at the onsite Conference Registration Desk. The desk will open on 23rd of March at 8:30 a.m. Upon registration onsite, the conference team will provide participants with information about the event and a badge, which should be worn visibly at all times. The personalised badge authorises participants to attend all conference sessions, and lunch and coffee breaks.

Cancellation Policy
Dravet Syndrome Foundation Spain reserves the right to cancel any registration forthwith and without liability on its part in the event of any damage to, or destruction of the venue by fire, shortage of labour, strikes, industrial unrest, public health measures, or any other cause beyond the control of the Dravet Syndrome Foundation Spain, which shall prevent it from performing its obligations. In these circumstances, every effort will be made to find an alternative format/venue or propose a new date for the conference. Should the conference be cancelled for any reason, airfares, hotels, or other costs incurred by the participants are not reimbursed by the Dravet Syndrome Foundation Spain.

Any participant is allowed to cancel his/her registration but must notify this to the conference organisers by e-mail at [email protected] within a minimum of 10 days prior the event. Failure to attend the event without prior written notification by email may incur a no-show fee, which shall cover individual organizational, material and catering costs.

Name Change
All registrations are only valid for the respective person mentioned in the form. A registered participant unable to attend the conference may nominate (free of charge) a substitute participant by notifying the conference organisers by email at [email protected] before 16th of March 2023. The procedure allows giving the last name, first name, and email address of the new participant.

Personal Data
Upon registration, the participant authorises Dravet Syndrome Foundation Spain to use their personal data stated above to inform the participant of any further conferences, events, or information concerning the conference and Dravet Syndrome Foundation Spain’s activities. The conference booklet online will only contain authors, titles, and affiliations of submitted abstracts, but no full abstract texts. The conference printed booklet will contain full abstract texts from all participants. Conference certificates will be sent to the participants as a PDF document upon request at [email protected]. Only the name & affiliation will be stated in the certificate, not the email addresses of the participants.

Information is collected by an automated computer system that manages registrations. Security and confidentiality are guaranteed by state-of-the-art technical tools. However, Dravet Syndrome Foundation Spain declines any responsibility concerning data security transmitted through the internet.

Photography
The conference is held in public; therefore, we do not ban participants, exhibitors, partners and other companies from taking photographs of conference activities and participants, and share it via social media. By participating in the conference, you approve that general pictures will be taken and used for our Social Media output, our websites and other communication purposes. You hereby waive any right to oppose, and expressly authorise the organiser to make them available on the Internet or as it may seem fit. Author’s presentation slides shall not be photographed by any conference participant.

By participating in the conference, you approve that general pictures will be taken and used for our Social Media output, our websites and other communication purposes. You hereby waive any right to oppose, and expressly authorise the organiser to make them available on the Internet or as it may seem fit. Author’s presentation slides shall not be photographed by any conference participant.

The conference organiser cannot be held responsible for any loss, injury or damage to any property, whatever the cause may be. Should, for any reason outside the organiser’s control (e.g., political or economic circumstances or a case of ‘force majeure’), the venue or speakers change, or the event be cancelled, the organiser will endeavour to reschedule, but shall not be held responsible for any costs, damages or expenses incurred by the participants. If for any reason the organiser decides to make material changes to this conference, the organiser is not responsible for airfares, hotels or other costs incurred by the participants. The participant takes part in the conference, all tours, and trips at his own risk.
